Patient Services Representative Responsibilities

• Responsible for maintaining all new and current patient accounts.

• Handle all patient inquiries regarding the program, appointments, and fees.

• Scheduling appointments for patients.

• Communicating with various teams regarding patient appointments.

• Ensuring patient accounts are current and in good standing.

• Ensuring billing information, including insurance information, is accurate and updated.

Position Summary: 

The Care Coordinator will provide care coordination, assessment, and treatment planning services to patients with a substance use disorder. Care coordination services mean those activities provided to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs. Care coordination  services at BrightView consist of: (1) assessment of the eligible patient to determine service needs; (2) development of an individualized integrated action plan; (3) referral and related activities to help the patient obtain needed services; (4) monitoring and follow-up; and (5) evaluation. 

Job Responsibilities: 

  • Connect patients and families with available resources and follow-up to document use/success of referral. 

  • Complete detailed reports on needs assessments and care coordination activities. 

  • Input accurate and complete case management data into electronic patient database. 

  • Assist patients in accessing appropriate medical, social, educational, and employment resources. 

  • Monitor and ensure patients are seeing health care professionals outside of Brightview when required by the BrightView treatment team. 

  • Provide crisis and de-escalation techniques. 

  • Assure documentation is completed in compliance with contract regulatory agency requirements and best practices. 

  • Ensure patients have reliable, consistent transportation to BrightView and other care providers. 

  • Coordinate patient care with other BrightView team members. 

  • Complete documentation within specified timeframe using Electronic Medical Records. 

  • Attend required meetings including individual and group supervision, treatment team meeting, and quarterly in-service. 

Skills & Qualifications

Job Qualifications: 

  • Bachelor’s degree in one of the following fields (Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling) and 2+ years of experience in providing services to those diagnosed with SUD
